Applying to private/independent colleges and universities:

Private/independent  colleges and universities establish their own application deadlines and the admissions application used. 

Many use the Common Application which is a single online college application form used by over 900 colleges and universities. Instead of filling out the same general information—like your address, GPA, and extracurriculars— a dozen times, you only have to do it once.

College Report Form 

If you need this completed, please follow the instructions below:

Students must complete the Student Section(s) of the College Report. Blank forms will not be accepted and will delay processing.

Students can email the completed form to Amy Coffey (acoffey@ccsf.edu) and cc studentlife@mail.ccsf.edu. They must include their CCSF ID number in the email. Request must be submitted no less than 14 business days before the deadline. Please include the due date in your email request.

Students must list the institution(s) and the contact information where the College Report should be sent to. All institutions accept electronic submissions; students must include the email address or website where their form(s) is to be sent. If it needs to be faxed, the fax number needs to be included (fax submissions will be done on Mondays and Wednesdays).
