This is your annual opportunity to make changes to your medical and vision plan, enroll/re-enroll into FSA and/or AFLAC: Voluntary Supplemental Insurance without having a qualifying event. Enrollments or changes made during Open Enrollment will take effect on January 1, 2023.

  • Review your current medical plan, other medical plans offered, and 2023 medical/vision rates

If you want to change your medical plan or want to know more about next year's benefits, review the 2023 CCSF Benefits Guide by visiting the SF Health Service System (SFHSS) website or call SFHSS at 628-652-4700

Enroll or make changes to your medical/vision plan through:

  • eBenefits self-service: For instructions to register and use eBenefits self-service, go to https://sfhss.org/ebenefits 
  • fax the completed 2023 Open Enrollment Form and any required documentation directly to SFHSS, fax # is 628-652-4701 or,
  • mail to SFHSS: 1145 Market Street, 3rd floor, San Francisco, CA 94103.

Note: Keep a copy of your forms, and if faxing confirmation keep the confirmation fax report

In December, SFHSS will send to you your 2023 enrollment confirmation letter, please make sure you review your letter. If you have any questions, call SFHSS directly at 628-652-4700.

Flexible Spending Accounts (FSA) Open Enrollment is from October 1 to November 30, 2022.

** Please Note: all current enrollees MUST re-enroll for the 2022 plan year to continue FSA Healthcare and/or FSA Dependent Care. Enrollment is for the plan year starting on January 1, 2023.

Note: Part-Time Faculty and Classified employees working less than 20 hours/week and As- Needed are not eligible for FSA.

Want to enroll? Self-enroll online through www.wageworks.com. If you do not have a Wageworks account, you will need to register for an account. Once you are registered, when you log in, select "Enrollment Options" for you to enroll in your desired program. Please note: When enrolling you should enter your annual election amount under “Election Amount”.

FSA accounts are voluntary plans. For additional plan features, FSA rules, or a list of eligible expenses, please visit www.wageworks.com, or call 1-877-924-3967.

Employees and eligible dependents who are enrolled into a medical plan with San Francisco Health Service System (SFHSS), will automatically be enrolled into the VSP Basic Vision Plan. Employee and dependents enrolled in an medical plan with SFHSS may elect the Premier VSP Plan.

VSP does not issue insurance cards. If you are enrolled into the VSP plan (group# 12145878), please call VSP at 1-800-877-7195 for your member ID number.

If you have a qualifying event during the year, you can add or drop your dependents from your benefits. You have within 30 days from your qualifying event to make changes to your benefits.

SFHSS administers the medical and vision plans. For information on Health Benefit Guide, Medical/Vision Premium Rates, Health Enrollment Form, and COBRA Medical Rate. please visit https://sfhss.org/benefits/city-college or contact SFHSS at 628-652-4700. How to Calculate Domestic Partner (DP) Premium:

  • For Employee + 1 DP: Employee + 1 subtract Employee Only premium
  • For Employee + 1 DP + 1 Dependent: Employee + 2 or more subtract Employee + 1 premium
  • For Employee + 2 DP or more: Employee + 2 or more subtract EE only premium

*** The DP premium is generally post tax unless you are qualified for the Federal Tax Exemption for Dependents Who Meet Certain Requirements or Equitable California State Tax Treatment and complete the SFHSS Domestic Partner Declaration Form. For information and SFHSS Domestic Partner Declaration Form, go to https://sfhss.org/domestic-partner-health-coverage-and-your-taxes. Consult with your professional tax advisor before completing the form.

Delta Dental is a PPO plan and is offered to eligible active employees and dependents at no premium cost. Delta Dental does not issue insurance cards. If you are enrolled into the our plan (group# 15935), please call Delta Dental at 1-866-499-3001 for your member ID number or use your Social Security number. You may also register for a Delta Dental account by going to www.deltadentalins.com. By having an account, you can:

  • Receive your Explanation of Benefits (EOB) or Pre-Estimate of dental services electronically
  • Review Plan Details
  • Verify who is covered under your dental plan (i.e. dependents)
  • View or print an ID card

Delta Dental is administered by City College of San Francisco.

Flexible Spending Accounts are voluntary plans. There are two different types of Flexible Spending Accounts (FSA): Healthcare FSA and Dependent Care FSA.

You need to determine the annual contribution you want to make to a Health Care account for health, vision or dental care...etc for you and your dependents, and/or Dependent Care account for daycare expenses. Keep in mind that you cannot transfer money between the two accounts, and you cannot transfer participation to your spouse. Only expenses incurred on or after your effective date of coverage are reimbursable. In accordance to district policy, your annual election will be divided by the number of pay periods (or remaining number of pay periods) according to your classification. 

Healthcare FSA is a pre-tax benefit account used to pay for eligible medical, dental, and vision care expenses that aren't covered by your insurance plan. Healthcare FSA minimum annual contribution is $50 minimum and the maximum contribution is $3,050 for 2023 calendar year. Unused Flexible Spending Account Health Care dollars up to $610 can be carryover to the following year 2024.  

Dependent Care FSA is a pre-tax benefit account used to pay for dependent care services, such as preschool, summer day camp or elder daycare. Dependent Care FSA minimum annual contribution is $50 minimum and the maximum contribution is $5,000 per calendar year ($2,500 if married and filing a separate tax return). 

If you have a qualifying event during the year, you can elect to enroll or change your benefits. You have within 30 days from your qualifying event to make changes to your benefits.

  • Part Time Faculty, and Classified employees working less than 20hours/week and As-Needed are not eligible for FSA
  • During Open Enrollment current enrollees MUST re-enroll for the next plan year to continue FSA election.
  • It is the employee's responsibility to log into the Wageworks website to review and track the employee's own FSA account (i.e. account balance).

 

Commuter Benefits is a pre-tax saving program to pay for public transportation to and from work and parking fee associated with work.  

Self-enroll anytime online through www.wageworks.com. You can change or cease the amount throughout the year. Go to your Wageworks account to maintain or update your enrollment. If you do not have an account you will need to register for an account.

Basic Life and Accidental Death and Dismemberment (AD&D) is offered to eligible employees at no premium cost. Coverage amount is $50,000 (under age 70) and coverage will reduce on the 70th and 75th birthday.

Beneficiar(ies) can be updated at any time by completing a Life Insurance Enrollment form. It is important to name beneficiaries to ensure that any benefits payable at your death are left to survivors you intended.

EAP provides confidential, voluntary, free counseling services to employees. Get one-on-one counseling for personal and work-related concerns form a licensed EAP therapist.

Eligible employee may enroll in voluntary benefits such as whole or term life, short term disability, cancer, critical care, accident, hospital, supplemental dental & vision through AFLAC at a discounted rate. Employees pay full cost of individual or family coverage via authorized payroll deductions.

Note: Classified employees working less than 20 hours/week and As-Needed are not eligible for AFLAC: Voluntary Supplemental Insurance.

New hires will receive an email from AFLAC to receive initial educational consultation and guidance.

Existing CCSF employees can make additions or changes to their AFLAC, pretax advantaged benefits during CCSF benefits open enrollment period. Existing policies are guaranteed renewable.

Effective July 1, 2022- June 30, 2023, SEIU Classified Employees working 20 or more hours per week and enrolled with CCSF's provided Health Service System health plan may receive drug co-pay reimbursement.

All receipts must be submitted to CCSF Benefits Unit no later than June 30, 2023.
