MISSION
To provide health care services to support
and promote the physical and psychological health of the
students at City College of San Francisco. As an integral
part of the Student Development Division, the professional
staff in nursing/medical and psychological services, and
the classified staff function collaboratively to promote
student retention, satisfaction, and achievement through
open and hospitable access, direct clinical care, consultation,
and referral to assist students in maintaining and promoting
mental and physical health within available resources.

All students who are currently enrolled in
credit classes at City College of San Francisco are eligible
for care. Current student ID and a picture ID must be shown
before receiving service.
We have the right to refuse services and due to limited
financial resources we may limit our medical and psychological
services. We will work with you to provide you with
referrals when appropriate.
Appointments

Appointments may be made in person at the Student
Health Center or by telephone (415) 239-3192. Some same-day
appointments may be available.
Confidentiality

All your Student Health Service medical & psychological
records are confidential. Information will NOT be released
without your written consent except for legal subpoenas and
some communicable diseases cases, which by law, must be reported
to the Department of Public Health. There are also other
legal situations where official agencies are involved that
information may be released (see Request for Medical Records
for more information).

Health Information
To protect your privacy and to abide by the
HIPAA federal regulations, you are required to present a
picture ID and a current student ID for every visit. On
your first visit you will be required to sign a confidentiality
statement.
Request for Medical Records

Your medical records are confidential except
under certain circumstances:
- You allow your medical information to be shared by
other medical care providers you designate by signing
a release of medical information form.
- There is a legal subpoena
- Certain communicable diseases must be reported like
TB, Hepatitis, syphilis, meningitis, etc.
- If we are concerned that you are suicidal or homicidal
we must report this for your own or public safety.
To have your medical records you need to do
the following:
- You must sign a release of medical information at CCSF
student health Services indicating where to send the
information. There will be a cost for medical record copying and handing.
- Go to the medical offices where you want your medical
records at CCSF to be sent and sign the release of medical
information form and mail it with $10 - $25 check.
- If you are out of state and need an immunization record
for school entrance, etc., send a check payable to City
College of San Francisco Student Health Services and
send your:
- Full Name
- Student ID or Social Security Number
- Birth Date
- Complete name and address where you want this
record sent.
