Employer's Information

Print the Employment application and Information material using links listed below.   THE INFORMATION MANUAL AND ALL TWELVE FORMS and INSTRUCTIONS MUST BE PRINTED TO PROCESS FOR NEW HIRE EMPLOYMENT When all forms are completed,  Please refer student to Freda Leeper in the CalWORKs Office, Bungalow 615.  Ms. Freda Leeper will accept the documents and forward them to Student Employment and Payroll Office for processing.  Ms. Freda Leeper will accept documents from Mondays through Thursdays 9:00 A.M. to Noon and 2:00 P.M. to 3:00 P.M.

Use the New Hire Student employment Application for new employees and The Department New/Rehire Employment Form  for both,  New hires and Rehire student employees.

If Rehire employee has been inactive in payroll system for one year (or) longer, the student must submit the current Tuberculin Test/X-Ray Documentation to Student personnel.   If Rehire employee has been inactive in payroll system for four years or longer , the New Hire paperwork must be submitted.

You need to ask the student if he/she has worked for another department at CCSF and process this type of student as Rehire (four year limitation still applies).  The students who worked for  Student Government, CCSF Bookstore, or Contracted Positions are considered a New Hire.
