STEP ONE

Attend a workshop or complete a workshop equivalent by either:

a) Viewing the "First Step to Success" Video (Phelan, R403)

b) Visiting the website: http://www.ccsf.cc.ca.us/Services/Counseling/

If you remain on probation for future semesters, you do not have to repeat the workshop requirement.

STEP TWO

After completing the workshop, make an appointment immediately to see a counselor or the counselor to whom you are assigned.

If you attended a workshop or viewed the video, please bring your "First Step to Success" booklet to your counseling appointment as proof of completion of the workshop requirement.

If you visited the website, you will be given directions at the website for receiving credit for completing the workshop requirement.

STEP THREE

If you're Subject to Dismissal, bring or mail your Academic Plan to the Associate Dean of Students (Science building, Room 1930) at the Phelan Campus and complete the Petition for Re-Admission on the reverse side of the Dean’s letter.

STEP FOUR

If you are on Academic Probation, you need to pull your TOTAL grade point average to a 2.0 or above.

If you are on Progress Probation, you will be removed by decreasing the percentage of "Withdrawal," "Incomplete" and "No Credit" notations on your report card to less than 50% of ALL units in which you have enrolled.
