| California Nonresident Tuition Exemption:

Spanish)

If you attended high school in California for
three or more years and graduated from a California high school or
attained the equivalent thereof. You may be exempted from payment of
the non-resident tuition in credit courses if you meet the above
mentioned criteria. If you do not possess legal immigrant status, you
must file a California Non-Resident Tuition Exemption Request (AB540),
stating that you will file the appropriate paperwork to legalize your
immigration status as soon as you are eligible to. You must also
provide the college with a copy of your high school transcript showing
three years of enrollment in a California high school, and graduation
from a high school in California.
You may apply at any time. Complete the California
Nonresident Tuition Exemption Request online, print it
out and submit it to the Residency Department located at City College
of San Francisco, 50 Phelan Avenue, Conlan Hall E107. The form is also
available at the following locations: the Admissions Office in Conlan
Hall Room E107; the Latino Services Network (Cloud Hall 364) and the
African American Scholastic Program (Bungalow 500); the Admissions or
Counseling Offices at all the campuses. |

| Change of Name/Address: |
| This form is used to change you Name or Address.
Must attach copies of your picture ID and Social Security Card. |

| Course Registration Form: |
| Sample form that allows you to add your courses
in person on or after your in person registration date. Original form
can be obtained form the Registration Center, Smith Hall Room 118. |

| Credit By Petition: |
Students who have attended noncredit classes at
City College of San Francisco and who wish to matriculate in a credit
program at City College of San Francisco to continue their education
for an Associate in Arts or Associate in Science degree may petition
the appropriate department at City College for credit upon achieving a
certificate in a specific noncredit certificate program. Units may be
applied toward the Associate degree after students have completed one
City College credit course with a value of at least two semester units
with a minimum grade point average (GPA) of 2.0. Students may obtain a
Credit by Petition form from the Campus where they obtained the
certificate. See the list below for eligible programs:

| Credit Card Payment Authorization Form: |
| To give CCSF authorization to charge your credit
card for tuition and enrollment fees. |

| Credit Card Refund Request Form: |
| Use this form to apply for an Enrollment Fee
Refund for a course dropped on or before the last day to drop for a
full refund. |
| |
| Pass/No Pass(Formerly Known as Credit/No
Credit) Grading Request: |
| Some courses are designated as optional Pass/No
Pass courses. Such courses are shown in the catalog with
the statement "P/NP available." meaning the course may be taken on
a Pass/No Pass or on a letter grade basis; i.e., the Pass/No
Pass
option is available. Students should be aware that Pass/No Pass
courses are counted toward graduation and for transfer and that in an
optional course a student must declare to the Office of Admissions and
Records the intention to take the course on a Pass/No Pass
basis no
later than the deadline date established in the Calendar of
Instruction. (See Calendar of Instruction for this date.) Forms to
exercise this option are available from the Office of Admissions and
Records. Students may also select "Pass/No Pass" grading option
on-line through WebSTARS at http://www.ccsf.edu.
In the absence of a "Petition for Pass/No Pass" students will
be
evaluated on a letter-grade basis. The "NP" symbol shall be used only
in calculating units attempted for probation and dismissal. |

| Course Repetition Petition: |
| You can obtain this form from Dean of Students
located at Conlan Hall Room 106. This form is used repeat a course that
you have previously passed or want to take again because of a lapse in
time. |

| Extension of Incomplete: |
| If a student is, due to extenuating
circumstances, unable to complete the work within the time specified
the student may, after contacting the instructor, petition the Dean of
Student Advocacy, Rights and Responsibilities for an Extension of an
Incomplete. |

| Evaluation of Course Equivalency: |
| This form is to be filled out by the student and
all documentation (e.g. course description) required for this
evaluation. |

| Graduation Requirement Waiver: |
| The general education graduation requirement for
Area G, Physical Skills and Health Knowledge, may be waived for those
students in semi-professional programs who have completed thirty
semester units at the College. A petition must be filed with the
program advisor, or the person who maintains these students' official
file. |

| Late Add: |
| Form used to add courses after the last day to
add a course has passed. |
| |
| Late Withdrawal: |
| Petition for a Late Withdrawal from a course.
Must be approved by office of Student Advocacy, Rights and
Responsibilities, located at Conlan Hall, Room 106. |
| |
| Leave of Absence: |
| This Leave of Absence will drop or withdraw you
from ALL classes in which your are enrolled this term. You will still
be liable for all fees if you are dropped after the deadline date for
dropping a course for a full refund. |

| Petition for Credit by Examination: |
| According to Section 55753 of Title 5 of the
California Code of Regulations, community college governing boards are
authorized to grant credit by examination, under certain circumstances.
Please refer to the catalog for further information. |

| Petition to Challenge a Prerequisite: |
| A student who wishes to challenge a prerequisite
or corequisite should file a petition as early as possible prior to the
first day of class, but no later than the end of the add/drop period.
The student shall bear the initial burden of showing that grounds exist
for the challenge. |

| Petition to Change Section/Course: |
After the official deadline to add a class has
passes, a student may change a course or section only if he/she is:
- Changing to a higher or lower level in
sequential course
- Changing to a different section of the same
course.

| Time Conflict Approval: |
| Petition to take two classes that overlap with
the beginning or ending time of a course. Must have both instructors

| A student may petition the Dean of Student
Affairs, to waive local college regulations. |
